Complete and return this form by October 1, 2021 along with your show dues ($125.00) and copy of your USEF approval (or if not yet available, a copy of your USEF date application until your show approval is received from the USEF).
If you are applying for dates for more than one show, a separate form should be submitted for each show. Show Date Applications submitted without payment will not be accepted. All managers of NorCal approved shows must be current NorCal members. Applications will be returned if the Show Manager listed is not a member.
Applications submitted for shows that include payment, and at least copy of the USEF date application will be listed as "Applied". In order for a show to listed as "Approved", a USEF license and completed Ratings and Officials Form must be submitted at least 90 days prior to the show.
